There is no universal 'one month is always fine' rule. Japan's ordinary personal-import thresholds distinguish prescription, poisonous or deleterious medicines, other medicines and qualifying external-use products. Exceeding the applicable range requires import confirmation, while controlled substances and certain restricted medicines need separate checks regardless of a short trip or small supply.[34][35]
Start with the person who will use the medicine
MHLW's personal-import route is for the importer's own use, not sale, gifts or combining other people's supplies. The controlled-drug guidance also requires the traveller to carry their own medicine rather than mail it or entrust it to relatives.[34][35]
Our advice: list each product's ingredients, strength, formulation, prescribed or labelled usage and total quantity against its actual user. This is border-preparation guidance, not advice on diagnosis, treatment or changing doses. If procedures affect treatment arrangements, discuss medical decisions with the prescriber and import requirements with the Japanese authority separately.
Classify the product before applying a quantity threshold
The Japanese MHLW page lists up to one month's supply, based on usage and dosage, for poisonous, deleterious or prescription medicines. Other medicines and quasi-drugs outside the preceding categories have a two-month threshold. External-use products excluding those poisonous, deleterious or prescription categories have a separate allowance of up to 24 standard-size units per item.[34]
These are conditional ranges for import without an import-confirmation certificate, subject to Customs checks, not universal permission. Products sold overseas as foods or supplements may count as medicines in Japan because of ingredients or medicinal claims.[34] Do not classify a product solely by its shop label.
Count the contents, not boxes or holiday days
MHLW illustrates two months using two tablets three times daily, multiplied by 30 days and two months, and says to count contents rather than boxes.[34] This demonstrates arithmetic; it does not give every medicine a two-month allowance or recommend that dosage.
Our advice: record the existing directions and actual tablets, doses or units carried, including reserves. A seven-day holiday does not turn several large packs into a seven-day supply. For as-needed treatment, combinations or different strengths, do not invent a daily dose: send the real prescription or product information to the authority and ask how to establish the applicable quantity.
Import confirmation is a separate administrative step
Quantities above the applicable personal-import range require an import-confirmation application. MHLW explains that import confirmation replaced the former Yakkan Shoumei procedure for the relevant imports from 1 September 2020 and directs applicants to the responsible regional bureau.[34] A prescription is not automatically a substitute for that process.
Our advice: identify the arrival airport, products, formulations and quantities before contacting the relevant office. Ask what documents and lead time apply. This article gives no universal processing-time promise for ordinary import confirmation. Reducing the quantity does not resolve a separate ingredient restriction, and an import-confirmation certificate must not be assumed to replace a controlled-drug permission.[34][35]
Controlled drugs require ingredient and category checks
The Narcotics Control Department says to identify the controlled category first; its substance list does not include brand names. Some medicines cannot be imported even when legally prescribed overseas. Medicines classified as narcotics or stimulants' raw materials require advance permission for entry or departure; taking leftovers out also requires attention to the export application.[35]
For that application route, the guidance says to apply at least 14 days before travel. It specifically warns that Tokyo NCD cannot accept last-minute applications, and contacting an office does not guarantee handling.[35] Start early; neither a short stay nor a foreign prescription establishes permission.
Psychotropics: check quantities and injections separately
For non-injectable psychotropics within its table's quantities, NCD says no doctor's certificate or authority permission is required under the Narcotics and Psychotropics Control Law. Above those quantities, or for injections, it says to carry a doctor's certificate. It separately directs travellers carrying more than a month's supply to the medicine-import contact.[35]
The Japanese MHLW page states that medical psychotropics exceeding one month's supply, or in injection form, require advance import confirmation.[34] Read these requirements together: no permission under one law does not remove other paperwork. This guide does not reproduce ingredient thresholds or declare any particular product exempt; confirm the actual ingredient, strength, formulation and total amount.
Devices have different rules; keep the verification date
MHLW separately lists one set for household medical devices and up to two months for disposable medical devices; ordinary individuals cannot import devices intended for professional medical use.[34] Do not extend the medicine rules to every device, syringe or consumable. List equipment separately and ask about its classification.
Both official texts were checked on 20 September 2026. Neither retrieved body gave a clear publication or revision date, recorded as unspecified. The 2020 date above concerns a procedure change, not page publication; an undated holiday notice is not a reliable update date. Recheck before departure. Airline security, liquid carriage, transit-country rules and onward-destination requirements were not verified here.
Before departure
- List ingredients, strength, formulation, directions and total quantity for the actual user; do not pool other people's supplies.[34][35]
- Classify first, then apply the relevant one-month, two-month or qualifying external-use threshold.[34]
- Count contents and reserves, not packs or holiday days; do not change treatment to fit a threshold.
- Check import confirmation above the applicable range and any restrictions unrelated to quantity.[34]
- Check controlled-drug permissions, medical documents and leftover medicine on departure; 14 days is an application recommendation, not guaranteed processing.[35]
- Check psychotropic table quantities, injections and import confirmation together; assess equipment separately.[34][35]
